Internet Services Agreement

The following notes should help you in completing the ZYTRAX Internet Services Agreement.

The number in the following sections relates to the line number on the left or when enclosed in parenthesis e.g. (31).

Company Details

1. Company Name

Enter your registered company name.

2. Address

Enter the full address at which service is to be provided.

3. Address (continued)

Enter any additional address information (e.g. suite number, floor, mail stop) for the address at which service is to be provided.

4. City, Province

Enter the City and Province in which service will be provided.

5. Postal Code

Enter the postal code of the location at which service will be provided.

6. Telephone Number

Enter the main telephone number of the location where service will be provided.

7. Fax Number.

Enter the fax number at the location where service will be provided.

Contacts

10. Technical & Services Contact

Enter the name of the most appropriate person who will be the primary contact on technical & service matters. We will use this contact name for any issues relating to service requirements, service operations and notify this person of any proposed technical changes to our services.

11 Telephone Number

Enter the telephone number (or extension if the same as item 5 above). of the Technical & Services contact identified in item 10.

12. Billing Contact

Enter the name of the most appropriate person who will be the primary point of contact for any matters relating to billing or invoicing. 

13. Telephone Number

Enter the telephone number (or extension if the same as item 5 above). of the billing contact identified in item 12.

Billing Information

20. Billing Address

Enter the billing address only if different from that supplied in items 2 - 5 above.

21. Billing Address (continued)

Enter any addition information (suite number, floor etc.) about the billing address only if different from that supplied in items 2 - 5 above.

22. City, Province

Enter the City and Province of the billing address only if different from that supplied in items 2 - 5 above.

22. Postal Code

Enter the Postal code of the billing address only if different from that supplied in items 2 - 4 above.

Internet Information

30. What is your Domain Name

If you have a registered Domain Name(s) enter it/them here. If you have none leave it blank.

31. Select a Domain Name 

If you already have a Domain Name (item 30) leave this blank. If you do not have a registered Domain Name enter the name you would like for your domain e.g. 'sloppy_joe'. 

Please note that at this stage in the development of the Internet the chances of you being able to register precisely the name you want are very low. For some domain types registration fees (both initial and recurring are applicable). The customer is responsible for payment of these fees directly to the registration organization involved. This link will take you to our Domain page.

32. Domain Types required

If you already have a Domain Name (item 30) leave this blank. If you do not have a registered Domain Name enter the types of Domain that you would like to register. The domain type is the 'dot something' after the domain Name e.g. sloppy_joe.com, the '.com' is the domain type. This link will take you to our Domain page.

Please note that for some domain types registration fees (both initial and recurring are applicable). The customer is responsible for payment of these fees directly to the registration organization involved.

33. Do you have ISDN lines now

If you already have ISDN lines installed or on order enter the numbers here. Each ISDN lines has two associated numbers, the order is important enter the Primary ISDN number first (identified by the telephone service provider). If you do not have ISDN lines enter 'NO'. Note that ISDN service is not always available and may depend on your address.

Services Required

We ask you to fill this information in to allow you to verify your costs before entering into an agreement with us. We will fill in any missing information before we return the agreement.

40 Service Code

Each service that we offer has a service code identifying it. Enter this number here.

41. Qty

Enter the quantity of each service required.

42. Price Each

Enter the price for each service as shown in the relevant price plan here. If you leave it blank, we will fill it in when we return the agreement.

43. One-time

Enter any one-time or set-up charges associated with the plan here.

44. Monthly

Multiply columns 41 * column 42 to give the monthly price and enter it here. If you leave it blank, we will fill it in when we return the agreement.

Access Services

50. Primary Access Plan

Enter the service code and other information relating to the Primary Access Plan here. Primary Access plans are Dial-XX, DialLite-XX. Fixed-XX, Office12-XX or PVA-XX.

51. Options (2 lines)

Enter the details of optional access services on these lines. Assume your Primary Access Plan is Fixed-64 and you wanted an additional 64K on demand the option Service Code would be DoD and quantity 1.

52. Static IPs

Enter the quantity of Static IP addresses that you need.

53. Add the totals in column 43 to give you an Access service subtotal.

E-Mail Services

If you are not using E-Mail services leave items 60 to 63 blank.

60. Email Plan

Enter the Service code of the E-mail plan you want in this line. If you are using the Mailhost service Qty should be used to identify the number of mail boxes you require.

61. E-Mail options

Enter the Service code and numbers of any E-mail options that you require.

62. E-Mail Total

Total column 43 and 44 to give you an e-mail services subtotal.

Optional Services

If you are not using any optional service  leave items 70 and 71 blank.

70. Router Rental

If you will be renting a router as part of the service enter the quantity and here.

71. Options Total

Total column 43 and 44 to give you an optional services subtotal.

Totals

80. Services

Total columns 43 and 44 to give a separate totals for all One-time and Monthly costs.

81. Prepayment

If you are prepaying for 12 months of service multiply column 43 for line 80 by 0.9 (to give a 10% discount). Multiply column 44 *12 * 0.9 (to give a 10% discount).

82.

Copy the number in line 81, if you are prepaying, or line 80 if not. The total amount you need to send us to start your service is the sum of column 43 & 44 on line 80.
